o Application and Environment where problem occurred - Prod, Test, Dev, etc.
o Page or Process where problem occurred - include identification if possible, i.e. process ID
o Is the problem global, specific system or isolated to a single or specific group of users
o Current status - i.e. System is down, performing poorly, intermintent, etc.
